The Record-Breaking Year: Measles in 2025

The U.S. recorded more than 2,200 confirmed measles cases in 2025, a number far greater than any year since measles was eliminated in the country. Texas experienced the largest outbreak, which lasted for nearly seven months. By August 2025, the state had declared the outbreak over, but not before it had caused widespread fear and led to preventable deaths. The spread did not stop there; outbreaks in South Carolina, Utah, and Arizona followed, with hundreds of new cases, many in schools, daycare centers, and community spaces.

As the first two weeks of 2026 began, 171 new cases were confirmed, nearly matching the average annual total of measles cases for the past 25 years. Experts agree that the frequency of new outbreaks and the alarming rate of transmission signal a serious public health crisis. Measles exposures were reported across a wide range of settings, including public schools, airports, churches, and restaurants. This widespread nature of the outbreaks only increases the difficulty of containing the virus.

The Threat of Losing Measles Elimination Status

The U.S. has held its measles elimination status since 2000, a significant achievement in the fight against infectious diseases. Measles was no longer considered endemic, and cases were few and far between. However, experts warn that the country may soon lose this status due to the increasing number of outbreaks.

In April 2026, the Pan American Health Organization (PAHO) will evaluate the U.S. measles elimination status, considering the epidemiological evidence from recent outbreaks. Losing this status would mark a significant failure in public health and a step backward in efforts to control vaccine-preventable diseases.

Dr. Caitlin Rivers, an epidemiologist at Johns Hopkins University, states that the frequency of measles cases is “startling.” What was once a rare disease is now becoming a frequent occurrence, and the situation shows no signs of improving unless urgent action is taken to curb the spread.

Vaccine Hesitancy: A Key Factor in the Resurgence

These deaths highlight the serious risks that measles still poses, especially to unvaccinated individuals and children.

In addition to the health risks, measles outbreaks place a significant burden on the healthcare system. Hospitals and healthcare providers are overwhelmed with patients, which strains resources and leads to delays in treatment. The economic costs of managing these outbreaks are also considerable, as healthcare providers must allocate significant resources to containing and treating the disease.

The rise in vaccine hesitancy can largely be attributed to the spread of anti-vaccine messaging. Dr. Paul Offit, a leading vaccine expert, notes that vaccines are “victims of their own success.” Because measles has been rare in recent years, many people have forgotten how deadly and contagious the disease can be. Without a visible reminder of the risks, people may not recognize the importance of immunization. Offit points out that many parents now view measles as something of the past and fail to understand the grave threat it still poses.

Combating Vaccine Hesitancy and Increasing Vaccination Rates

The solution to the measles resurgence lies in increasing vaccination rates across the country. Public health experts agree that vaccinations are the most effective way to prevent the spread of measles and protect both individuals and communities. Strengthening school vaccination requirements is one effective way to ensure that children are vaccinated before attending school, which can help curb outbreaks in educational settings.

In addition to stronger requirements, public health campaigns must focus on educating parents about the safety and importance of vaccines. Misinformation about vaccine safety must be countered with accurate, evidence-based information. Trusted community leaders, including pediatricians, public health experts, and local influencers, can play a vital role in dispelling myths and encouraging parents to vaccinate their children.

The resurgence of measles in the U.S. is a public health emergency that can be stopped through increased vaccination efforts and better public health messaging. By strengthening vaccination coverage, addressing vaccine hesitancy, and combating misinformation, the U.S. can regain control over the disease and prevent further outbreaks. Public health experts believe that with the right efforts, the U.S. can return to its previous status of measles elimination and ensure that children are protected from this preventable disease.
